A West Virginia coal mine is accused of 45 violations after two fatal accidents.
After the investigation into one of the deaths at Pocahontas Coal's Affinity mine, inspectors found new equipment installed improperly.
The inspector called it a serious violation, indicating what they are calling an 'extremely high degree of negligence.'
Blountville, Tennessee-based United Coal owns Pocahontas Coal. We're told they did not want to comment right away but did say they are cooperating with investigators.
